Triad DEXA provides body composition assessments using Dual-Energy X-ray Absorptiometry (DEXA). Services may include:

Online intake forms and consent waivers;
Physician approval of scan requests, where required by law;
Sale of scan packages through WooCommerce;
Secure scheduling of appointments through SimplyBook.me.

Important: DEXA scans offered through Triad DEXA are non-diagnostic and are provided solely for body composition analysis. Medical interpretation, recommendations, or treatment decisions are provided only by the prescribing physician in accordance with state regulations.

3. Eligibility

You must be at least 18 years old to use the Services. Minors may only undergo scans with written parental or guardian consent. Scans cannot be performed during pregnancy.

Triad DEXA is not a medical provider. Results of scans are for informational purposes only. You are responsible for consulting your physician regarding interpretation of results and any medical decisions. By using our Services, you acknowledge that Triad DEXA does not provide medical advice or treatment.

5. User Responsibilities

You agree to:

Disclose medical conditions or pregnancy status that may affect your eligibility;
Follow all scheduling, preparation, and safety instructions;
Use the Site only for lawful purposes and not attempt to interfere with its security or functionality.

6. Ordering, Payment, and Credits

Scan packages are purchased through our online store.
Payment is required before scans can be scheduled.
Packages grant a set number of scan credits, which may be redeemed for scheduling appointments.

7. Scheduling and Cancellations

Appointments are scheduled through SimplyBook.me. You are responsible for attending your scheduled time. Cancellations and rescheduling must comply with our posted cancellation policy. Missed appointments may result in forfeited credits.

We may share your scan results securely with your referring physician for review and approval.

9. Assumption of Risk & Release of Liability

By purchasing and using our Services, you acknowledge:

The DEXA scan involves low-dose ionizing radiation;

Triad DEXA, its staff, and affiliates are not liable for any injury, adverse reaction, or damages related to the use of Services, except as prohibited by law.
