1. Overview

Triad DEXA offers body composition scan packages (“Packages”) that provide a set number of scan credits. This Refund & Cancellation Policy explains how refunds, reschedules, cancellations, and expiration of credits are handled. By purchasing a Package, you agree to the terms outlined below.

2. Refunds on Purchases

Before Intake Approval: Packages may only be purchased after your intake form is approved. Purchases made in error will be reviewed on a case-by-case basis.
Within 7 Days of Purchase: You may request a full refund of unused packages/credits within 7 days of purchase.
After 7 Days: All sales are final. Refunds will only be considered in cases of error, duplicate purchase, or extenuating medical circumstances (documentation required).
Used Credits: Once a scan credit has been redeemed, it is non-refundable.

3. Credits & Expiration

Each Package grants a defined number of scan credits (e.g., 1, 2, 4, or 6).
Credits are non-transferable and associated only with the purchasing client.
Credits expire 12 months from the date of purchase, unless otherwise specified at the time of sale. Expired credits are forfeited.

4. Appointment Scheduling

Scans must be scheduled using our online booking system (SimplyBook.me).
A valid scan credit is required to confirm a booking.
You must complete all pre-scan intake and safety requirements prior to your appointment.

5. Appointment Cancellations & No-Shows

Cancellations/Reschedules: You may cancel or reschedule up to 24 hours before your appointment without penalty. Your scan credit will be returned to your account.
Late Cancellations (<24 hours): Credits may be forfeited for late cancellations unless due to medical emergencies or extenuating circumstances.
No-Shows: Failure to attend a scheduled appointment without prior cancellation will result in the loss of the scan credit.

6. Technical or Facility Issues

If a scan cannot be completed due to technical issues, equipment malfunction, or facility closures, your credit will be returned and you will be rescheduled at no additional cost.
